Welcome from Schenectady County’s Long Term Care Medicaid Unit. Our staff responds to consumers with such services as nursing assessments, case management supports and Medicaid eligibility as well as other services, including consumer directed services. The goal of the Long Term Care Unit is to allow long-term care Medicaid consumers to remain safely in their own homes as long as possible, instead of relying on institutional care. Additionally, we provide these same services for people who are enrolled in special waiver programs for Traumatic Brain Injury (TBI), Office for People With Developmental Disabilities (OPWDD), Nursing Home Transition Diversion (NHTD), and Care at Home (CAH).
Our Unit provides an access point for agencies and the community to obtain information about community based services. We review the costs and payment services, and when appropriate, refer to other services and agencies. The programs that are available through our Unit are for approved DSS Medicaid services. This can include personal care, light housekeeping, laundry assistance, grocery shopping, Personal Emergency Assistance Services (PERS) and other errands. The Consumer Directed Personal Assistance Program provides the Medicaid consumer the same services just mentioned, but allows for greater flexibility and freedom of choice to direct one’s own care.
